WAIVER OF LIABILITY AND INDEMNITY FORM AGREEMENT TO PARTICIPATE IN NORTHSIDE MARINE ‘ON WATER DAY’

I acknowledge and agree that:

a) Boating can be dangerous activity and my participation in this activity may expose me to danger.
b) I have agreed to participate in the activity with the knowledge of the risks involved in participating.
c) I will obey all directions given to me by or on behalf of Northside Marine.
d) I unconditionally waive my right to file action(s) or claim(s) which I may have against Northside Marine employees now or in the future arising from my participation in the activity.
e) I/we have read this document and understand it and are aware of the risks and danger of this activity.
